 Membership Application Form: ANZAP MEMBERSHIP FORM: Download PDF Application Form:DIPLOMA OF ADULT PSYCHOTHERAPY: Download PDF Course Fees Course application fee is $325 with $250 refunded if not called to interview. Annual fee for the Sydney course is $6000, payable in 4 instalments etc. This course fee includes attendance at all Saturday seminars and conference workshops, $100 subsidy for the Retreat, reduced fee for the ANZAP annual conference, associate membership of ANZAP (includes receiving the ANZAP Bulletin). Fees for individual supervision and personal therapy will be in addition. Fees for courses held outside Sydney vary and will be advised.
